EVALUATION AND MANAGEMENT OF THE DIZZY PATIENT

Evaluation and management of patients with vertigo and chronic dizziness is a challenging task to the practicing NeurOtologist. The main objective of this course is to present a cost-effective office approach to patients with vertigo and dizziness. This approach is physician directed and is based on thorough understanding of the anatomy, physiology, and pathophysiology of inner ear diseases. Detailed initial history is very important in directing office examination and obtaining additional audiological, vestibular, radiological, and biochemical tests. Structured and complete office neurotologic examination should include ear examination, tuning fork tests, and Frenzel’s glasses or video observation/recording of spontaneous and stimulated eye movements. The integrity of the vestibulo-spinal system can be screened using high compliant foam pads a. Audiological and Vestibular studies are outlined and discussed. These include basic and advanced Audiometry (ABR, ECoG, OAE), ENG, Dynamic VOR testing, and Dynamic Posturography. Differential diagnosis of inner ear and CNS causes of dizziness will be outlined and discussed. Office based medical and rehabilitation treatments for common inner ear diseases are discussed. This includes repositioning maneuvers for BPPV and home vestibular rehab for motion sickness, bilateral vestibular loss, and chronic dizziness.
